Area of Science:

  • Neurology
  • Neuroimmunology

Background:

  • Brainstem encephalitis is a rare neurological condition.
  • Differential diagnosis is crucial for appropriate management.

Observation:

  • Seven patients presented with a flu-like prodrome followed by eye movement disturbances and ataxia.
  • Cranial nerve deficits, limb dysmetria, and pyramidal signs were also observed.
  • Neuroimaging revealed brainstem swelling, and CSF analysis showed pleocytosis.

Findings:

  • The cases were diagnosed as Bickerstaff's encephalitis, a benign, post-infectious syndrome.
  • Cerebrospinal fluid analysis excluded multiple sclerosis.
  • Serological tests indicated possible viral triggers like Adenovirus or Echovirus.

Implications:

  • Bickerstaff's encephalitis is a distinct clinical entity with a favorable prognosis.
  • Early diagnosis based on clinical and diagnostic findings is important.
  • Understanding the post-infectious etiology may guide future research and treatment.
