Summary

Small mesencephalic hematomas can cause oculomotor paralysis. These cases highlight that computed tomography can identify these lesions, which often resolve with conservative management.

Background:

  • Mesencephalic hematomas are rare and can present with diverse neurological deficits.
  • Oculomotor nerve (cranial nerve III) palsy is a potential consequence of mesencephalic lesions.

Observation:

  • Two patients presented with unilateral oculomotor paralysis and mild neurological symptoms.
  • One patient exhibited pupillary abnormalities, mimicking a peripheral third nerve lesion.

Findings:

  • Computed tomography (CT) revealed small, unilateral anterior mesencephalic hematomas affecting the oculomotor fascicle.
  • Angiography and surgical exploration excluded arteriovenous malformations or neoplasms as the cause.

Implications:

  • CT is valuable for diagnosing small mesencephalic hematomas.
  • Conservative management appears effective, with significant clinical improvement observed within 3 months.
