Background:

Phlegmonous gastritis is a rare, potentially fatal infectious stomach disease.
Endoscopic findings are often nonspecific, complicating diagnosis.
Differential diagnosis includes scirrhous gastric cancer, gastric syphilis, and acute gastric mucosal lesions.

Observation:

Three cases of phlegmonous gastritis are presented with unique endoscopic findings.
Initial diagnoses included scirrhous gastric cancer, gastric syphilis, and acute gastric mucosal lesion.
Histopathology and cultures confirmed phlegmonous gastritis in all cases.

Findings:

Antibiotic therapy was effective in all three cases.
Case 1: diagnosed after suspected scirrhous gastric cancer.
Case 2: diagnosed after suspected gastric syphilis, with Streptococcus viridans identified.
Case 3: diagnosed after suspected acute gastric mucosal lesion, with Pseudomonas aeruginosa and Streptococcus viridans identified.

Implications:

Accurate early diagnosis of phlegmonous gastritis is essential.
Clinicians should consider phlegmonous gastritis in differential diagnoses despite varied endoscopic views.
Prompt antibiotic treatment can lead to significant improvement or resolution of gastric lesions.
