Summary

Vessel density in the radial peripapillary capillary network is reduced in patients with chiasmal compression optic neuropathy. This reduction further decreases after decompressive surgery, indicating progressive damage.

Background:

  • Chiasmal compression due to brain tumors can lead to optic neuropathy (ON).
  • Assessing the microvasculature of the optic nerve head is crucial for understanding ON progression.

Purpose of the Study:

  • To evaluate the vessel density (VD) of the radial peripapillary capillary (RPC) network in eyes with chiasmal compression ON.
  • To compare VD before and after decompressive surgery with healthy controls using swept-source optical coherence tomography angiography (SS-OCTA).

Main Methods:

  • A cross-sectional study involving 12 patients with chiasmal compression and 16 healthy controls.
  • Neuro-ophthalmological examinations including visual field testing, RNFL and GCC thickness measurements (SD-OCT).
  • Peripapillary VD was measured using SS-OCTA (Cirrus Plex Elite 9000) in four sectors.

Main Results:

  • Baseline VD was significantly lower in patients with chiasmal compression ON compared to controls (p=0.003).
  • Postoperative VD was significantly decreased compared to preoperative VD (p=0.004).
  • Preoperative VD correlated with RNFL thickness, GCC thickness, and visual field defects.

Conclusions:

  • Vessel density of the RPC network is reduced in chiasmal compressive ON.
  • Decompressive surgery resulted in a further decrease in RPC network VD at 6 months post-surgery.
