Asthma: Pathogenesis and Management

836
Asthma is a chronic pulmonary condition involving inflammation of the airways, hyper-reactivity, and reversible obstruction of the airways. This condition can significantly impact a person's quality of life, making breathing difficult and leading to distressing symptoms.
Asthma is classified as allergic and non-allergic. Allergens such as dust mites, pollen, and pet dander trigger allergic asthma, while factors like cold air, intense emotions, or exercise can induce non-allergic asthma.

Asthma-I: Introduction

2.9K
Asthma is a chronic respiratory ailment that requires careful management due to its varying symptoms and influencing factors. It is characterized by airway inflammation, bronchial hyperresponsiveness, and reversible airflow obstruction, leading to symptoms like wheezing, shortness of breath, chest tightness, and coughing. The symptom frequency and intensity may vary considerably over time. It is also linked to immune system responses to allergens and irritants, highlighting the complex...

Area of Science:

  • Pulmonary Medicine
  • Medical Education
  • Health Services Research

Background:

  • Asthma management requires continuous clinical attention due to its significant morbidity and mortality.
  • Physician knowledge is crucial for effective asthma care.
  • Gaps in physician knowledge and practice hinder optimal asthma management.

Purpose of the Study:

  • To identify and analyze the discrepancies in physicians' knowledge and practices regarding asthma management.
  • To assess the current understanding and application of asthma guidelines among physicians in Jordan.

Main Methods:

  • A cross-sectional online survey was conducted with 271 physicians in Jordan.
  • The Physicians' Practice Assessment Questionnaire (PPAQ), based on Global Initiative for Asthma (GINA) guidelines, was used.
  • Logistic regression analysis identified predictors of good practice.

Main Results:

  • Physicians demonstrated over 78% overall knowledge but had gaps in recognizing asthma attack signs (61.9%) and knowing appropriate drugs (67.6%).
  • Physician practice application averaged only 57.1% ± 25.7%.
  • Seeing 6-10 asthma patients daily, being a senior physician (>50 years), or seeing 1-5 patients daily were associated with better guideline adherence.

Conclusions:

  • A significant gap exists between physician knowledge and practice in asthma management.
  • Professional education, reminders, and clinical audits are needed to improve practice.
  • Novel knowledge transfer strategies are essential to change physician behavior and enhance asthma care.
