Summary

Topical and intralesional mitomycin C effectively treat ear keloids. Topical application showed better results and higher patient satisfaction compared to intralesional injections for keloid management.

Background:

  • Keloids are raised, fibrous scars extending beyond wound borders.
  • Auricular keloids present a common cosmetic and functional challenge.
  • Effective management strategies for keloids are continuously sought.

Purpose of the Study:

  • To compare the efficacy of topical versus intralesional mitomycin C for auricular keloid treatment.
  • To evaluate patient outcomes and satisfaction following two different mitomycin C administration methods.
  • To assess the impact of mitomycin C on keloid severity and recurrence.

Main Methods:

  • A prospective randomized study involving 40 patients with auricular keloids.
  • Group I: Surgical removal followed by topical mitomycin C (32 patients).
  • Group II: Surgical removal followed by intralesional mitomycin C injection (8 patients).

Main Results:

  • Both topical and intralesional mitomycin C significantly reduced the Vancouver Scar Scale (VSS) scores (p<.001).
  • No significant differences in patient or lesion criteria were observed between groups (p>.05).
  • The topical mitomycin C group demonstrated superior VSS improvement and higher patient satisfaction.

Conclusions:

  • Both topical and intralesional mitomycin C are effective in managing auricular keloids post-surgery.
  • Topical mitomycin C application offers better VSS scores and enhanced patient satisfaction.
  • Topical mitomycin C is a promising alternative for auricular keloid treatment.
