Background:

  • Upper gingival squamous cell carcinoma (UGSCC) is a subset of oral cancer.
  • Accurate prediction of regional lymph node (LN) metastasis is critical for effective treatment planning and prognosis in UGSCC.

Purpose of the Study:

  • To identify specific tumor characteristics associated with regional lymph node metastasis in UGSCC.
  • To determine key predictors for LN metastasis to guide clinical management.

Main Methods:

  • Retrospective analysis of 113 UGSCC patients from Osaka University Dental Hospital.
  • Evaluation of primary tumor dimensions (width, length, depth), bone invasion, T1/T2 signal intensity, tumor location, and T classification.
  • Histopathological analysis and assessment of LN metastasis status.

Main Results:

  • Bucco-lingual tumor width, histopathological classification, and tumor location were significant predictors of LN metastasis in both univariate and multivariate analyses.
  • Superior-inferior depth and T classification showed significance only in univariate analysis.
  • Tumor signal intensity was not a significant prognostic factor.

Conclusions:

  • Bucco-lingual width, histopathological grade, and tumor location are important predictors of LN metastasis in UGSCC.
  • These factors should be considered in the clinical management of patients with UGSCC to optimize treatment strategies.
