Summary

This study reports the first case of odontogenic carcinosarcoma with dentinoid, a rare jaw tumor. The patient fully recovered after surgical resection with no signs of recurrence after four years.

Background:

  • Odontogenic carcinosarcoma (OCS) is an exceptionally rare malignant odontogenic tumor.
  • OCS is characterized by the simultaneous presence of malignant epithelial and mesenchymal components.
  • Most reported OCS cases occur in the upper and lower jaws.

Observation:

  • This report details a unique case of OCS with dentinoid in a 58-year-old woman affecting the left maxilla.
  • The tumor exhibited extensive invasion and resulted in the loss of the left maxillary molar.
  • Histopathological examination revealed a triphasic tumor composed of malignant epithelium, malignant interstitium, and dentinoid, indicating bidirectional differentiation.

Findings:

  • The patient underwent successful nasal endoscopic sinus tumor resection.
  • Post-operative recovery was uneventful.
  • A rigorous four-year follow-up showed no evidence of disease recurrence or local relapse.

Implications:

  • This case represents the first documented instance of OCS featuring a dentinoid component.
  • The study provides a comprehensive description of the clinical, morphological, and immunohistochemical features of this rare entity.
  • This work aids in differentiating OCS with dentinoid from other related jaw malignancies.
