Summary
Liposarcomas of the spermatic cord are extremely rare malignant tumors. Radical orchiectomy is the standard treatment, but long-term follow-up is crucial due to frequent recurrences.

Background:
- Liposarcoma, a rare malignancy originating from fat tissue, can affect the spermatic cord.
- These paratesticular sarcomas account for 3%-7% of all sarcomas in the region and are often misdiagnosed preoperatively.
Observation:
- A 66-year-old male presented with a painless, slow-growing scrotal mass.
- Imaging revealed a 60x85x60 mm mass in the left inguinoscrotal region without abdominal or lymph node involvement.
Findings:
- The patient underwent radical inguinal orchiectomy as treatment.
- Liposarcomas of the spermatic cord present as palpable inguinal or scrotal masses.
Implications:
- Radical orchiectomy with high spermatic cord ligation and wide excision is the gold standard treatment.
- Long-term follow-up is mandatory due to the high rate of recurrence, even years after initial therapy.
