Summary

This study demonstrates that a tissue-engineered esophageal graft seeded with autologous mesenchymal stromal cells can regenerate esophageal tissue after full-thickness resection. This innovative approach shows promise for complex esophageal reconstructions.

Background:

Purpose of the Study:

  • To evaluate the feasibility of using a tissue-engineered esophageal graft for human segmental reconstruction.
  • To assess the regenerative capacity of the Cellspan esophageal implant (CEI) in a patient.
  • To report on the outcomes of an investigational treatment for esophageal cancer.

Main Methods:

  • Autologous adipose-derived mesenchymal stromal cells (Ad-MSCs) were isolated, expanded, and seeded onto a polyurethane scaffold (Cellspan).
  • The resulting Cellspan esophageal implant (CEI) was implanted in a patient following a 4 cm circumferential esophageal resection.
  • The CEI graft was sutured to the native esophagus using end-to-end anastomosis.

Main Results:

  • A clinical-grade, tissue-engineered esophageal graft was successfully used for segmental reconstruction in a human.
  • Histologic analysis 7.5 months postmortem showed complete luminal epithelialization.
  • Partial regeneration of esophageal tissue was observed in the implanted graft.

Conclusions:

  • Autologous Ad-MSC-seeded CEI grafts stimulate tissue regeneration after esophageal resection.
  • This case report supports the potential of tissue-engineered grafts for esophageal repair.
  • The findings highlight a novel therapeutic strategy for complex esophageal defects.
