Nephrotic Syndrome I : Introduction01:24

Nephrotic Syndrome I : Introduction

131
Nephrotic Syndrome is a chronic kidney disorder defined by clinical findings such as severe proteinuria, hypoalbuminemia, hyperlipidemia, and edema. These symptoms result from damage to the glomeruli, the kidney’s filtering units, increasing their permeability to proteins.Definition and Meaning:Proteinuria, defined as the loss of more than 3.5 grams of protein per day in adults, is a crucial feature of nephrotic syndrome. This condition is often accompanied by edema, the accumulation of...

Nephrotic Syndrome II : Assessment and Medical Management

37
IntroductionNephrotic syndrome is a kidney disorder marked by excessive protein loss in the urine, leading to various systemic complications. This condition often results from damage to the glomeruli—the kidney's filtering units—causing proteinuria, low blood protein levels, and fluid retention. Understanding the assessment, diagnosis, and management of nephrotic syndrome is essential for effective treatment and prevention of further kidney damage.AssessmentPatient History: Document...

Proteoglycans

4.1K
Glycans, a class of complex heterogeneous molecules, can be covalently attached to proteins to form glycosylated proteins that regulate various physiological and pathological processes. Glycosylated proteins or glycoproteins comprise N-linked and O-linked oligosaccharides. O-glycosylation is the most common type of protein glycosylation. Here, glycans attach to the oxygen atom of the hydroxyl groups of Serine or Threonine residues. O-linked glycosylation occurs later in protein processing,...

Chronic Kidney Disease III: Interprofessional Care

129
Chronic kidney disease (CKD) requires collaborative and comprehensive management. CKD progresses through stages and can lead to end-stage kidney disease (ESKD) if untreated. Interprofessional collaboration and patient education are crucial, enabling patients to manage their health and improve their quality of life.Diagnostic approach for chronic kidney diseaseThe diagnosis of CKD primarily focuses on the glomerular filtration rate (GFR), which assesses kidney function by measuring how well...

The glomerulus and Bowman's capsule are two essential components of the nephron, which is the functional unit of the kidney. These microscopic structures play a critical role in the process of blood filtration to produce urine.
Glomerulus: Structure and Function
The glomerulus is a tiny, intricate network of capillaries located at the beginning of the nephron. It's enveloped by the Bowman's capsule and receives its blood supply from an afferent arteriole, which divides into numerous...

Acute Kidney Injury II: Pathophysiology

239
Acute kidney injury (AKI) causes are categorized into three primary categories based on the location of the injury: prerenal, intrarenal (or intrinsic), and postrenal causes. This classification guides clinical management and illustrates how different pathways can impair kidney function.Etiology and Pathophysiology of Acute Kidney Injury1. Prerenal causesEtiology: Prerenal Acute Kidney Injury, the most common type, occurs when reduced blood flow to the kidneys decreases filtration capacity...

Area of Science:

  • Nephrology
  • Immunology
  • Biochemistry

Background:

  • Polymeric immunoglobulin A (Poly-IgA) immune complex formation and glomerular deposition are central to IgA nephropathy pathogenesis.
  • Current diagnostic methods for IgA nephropathy lack a specific, one-step serologic test for Poly-IgA levels.

Purpose of the Study:

  • To develop and validate a novel, one-step ELISA method for the serologic detection of Poly-IgA immune complexes.
  • To assess the clinical utility of this assay in distinguishing IgA nephropathy patients from healthy controls and those with other kidney diseases.
  • To evaluate the association of Poly-IgA levels with disease severity, pathological parameters, and treatment outcomes.

Main Methods:

  • A novel Enzyme-Linked Immunosorbent Assay (ELISA) was developed utilizing recombinant CD89 as a capture probe for plasma Poly-IgA immune complexes.
  • Semiquantitative measurements of Poly-IgA indices were performed on patient samples from Peking University First Hospital.
  • Longitudinal trends, associations with pathological parameters, treatment responses, and molecular composition via mass spectrometry were analyzed.

Main Results:

  • Significantly higher Poly-IgA immune complex levels were observed in IgA nephropathy patients compared to healthy controls and non-IgA nephropathy controls (P<0.001).
  • Elevated Poly-IgA levels correlated with reduced estimated glomerular filtration rate (eGFR) and poorer kidney outcomes.
  • The Poly-IgA index demonstrated good diagnostic discrimination (AUC, 0.78), outperforming galactose-deficient IgA1 levels (AUC, 0.70).
  • Treatment with corticosteroids and immunosuppressants led to a reduction in Poly-IgA indices.

Conclusions:

  • The developed recombinant CD89-based ELISA provides a sensitive method for detecting Poly-IgA immune complexes.
  • Higher Poly-IgA immune complex levels are associated with IgA nephropathy severity and predict treatment response to immunosuppressive therapies.
