This case report details a rare instance of pulmonary chordoma, a neoplasm originating from notochord remnants. Diagnosis was confirmed via surgical resection and immunohistochemistry in a patient with bilateral lung nodules.

Background:

Chordomas are rare bone cancers originating from notochordal remnants.
Most chordomas present in the axial or extra-axial skeleton, with pulmonary involvement being exceptionally rare.

Observation:

A 61-year-old woman presented with bilateral pulmonary nodules discovered during pre-bariatric surgery screening.
Surgical resection via video-assisted thoracoscopic wedge resection was performed for these nodules.

Findings:

Immunohistochemical analysis confirmed the pulmonary nodules as chordomas.
This finding represents a rare presentation of chordoma in the lung.

Implications:

The case highlights the importance of considering rare diagnoses in pulmonary nodules.
Surgical resection is indicated for chordomas due to their general chemoresistance, though further research into pulmonary chordoma treatment is warranted.
