Summary

Preoperative antibiotics are crucial for supporting host defenses and reducing surgical site infections (SSI) in breast cancer patients. Optimizing the local wound environment also significantly minimizes SSI risk during oncologic surgery.

Area of Science:

  • Oncology
  • Infectious Disease
  • Surgical Innovation

Background:

  • Surgical site infections (SSI) are influenced by pathogens, wound environment, and host defenses.
  • Cancer treatments can impair host defenses, increasing SSI risk in patients undergoing oncologic surgery.
  • Controlling patient risk factors can reduce SSI likelihood, potentially masking the need for preoperative antibiotics.

Purpose of the Study:

  • To clarify evidence supporting preoperative antibiotic use to bolster host defenses in breast cancer surgery patients.
  • To explore the role of wound dressings in the local wound environment after cancer surgery.
  • To evaluate the impact of different wound dressings on healing and SSI incidence in dermatologic oncologic surgery.

Main Methods:

  • Review of evidence on preoperative antibiotic administration in breast cancer surgery.

  • Analysis of studies comparing gauze dressings versus advanced wound dressings.
  • Comparative study of antibiotic-free silicone dressings versus triple antibiotic dressings in Mohs surgery.
  • Main Results:

    • Preoperative antibiotics are supported as essential for host defense in breast cancer surgery.
    • Gauze dressings may increase SSI incidence by acting as foreign bodies.
    • Antibiotic-free silicone dressings showed comparable healing and SSI rates to antibiotic dressings in Mohs surgery.

    Conclusions:

    • Preoperative antibiotics are vital for managing SSI risk in oncologic surgery.
    • The local wound environment, including dressing choice, is a critical factor in minimizing SSI.
    • Optimizing wound dressings can be as effective as antibiotics in certain dermatologic oncologic procedures.
