Background:

  • Titanium bridges are crucial for thyroid ala separation during type 2 thyroplasty for adductor spasmodic dysphonia.
  • Previous revision surgeries indicated potential failure of the bridge wings near the medial hole.

Purpose of the Study:

  • To investigate the incidence and causes of titanium bridge malfunctions.
  • To develop an improved titanium bridge design to prevent device failure.

Main Methods:

  • A questionnaire survey assessed surgeries and revision cases in Japan (2002-2014).
  • Scanning electron microscopy (SEM) analyzed fracture surfaces of retrieved titanium bridges.

Main Results:

  • Titanium bridges were used in 385 patients; 19 required revision surgery.
  • Eleven cases of wing breakage at the medial hole were identified, linked to fatigue fractures from bending stress.
  • No recurrence or adverse events were associated with the fractures.

Conclusions:

  • Device specifications were updated (oval hole, increased wing thickness) based on failure analysis to enhance durability.
