Area of Science:

  • Neurology
  • Infectious Diseases
  • Movement Disorders

Background:

  • Epilepsia partialis continua (EPC) presents as continuous focal motor seizures.
  • Distinguishing EPC from other repetitive movement disorders can be challenging.

Purpose of the Study:

  • To report a case of EPC in a young HIV-positive male with syphilis.
  • To highlight the utility of a simple mechanographic test in diagnosing EPC.

Main Methods:

  • Case report of a patient with right-hand EPC.
  • Utilized a pen and paper test to record movement frequency, amplitude, and rhythmicity.
  • Correlated clinical findings with neuroimaging of a left frontal lesion.

Main Results:

  • The patient exhibited right-hand EPC associated with a left cortico-subcortical frontal lesion.
  • Mechanographic data provided objective parameters of the involuntary movements.
  • The findings helped differentiate EPC from other potential causes of repetitive hand movements.

Conclusions:

  • EPC can occur in the context of HIV and syphilis.
  • Simple bedside tests can offer valuable diagnostic information for focal motor seizures.
  • Early and accurate diagnosis of EPC is crucial for appropriate management.
