Background:

Functional rating scales are crucial for tracking knee recovery but are difficult to find and compare.
Lack of awareness and accessibility hinders the selection of optimal tools for clinical practice.

Approach:

A systematic review identified and analyzed validated knee functional rating tools and their validation studies.
Methodological quality was assessed using QUADAS-2 and COSMIN-RB criteria.
An operational document was created detailing scale parameters, implementation, and interpretation.

Key Points:

73 studies evaluated 41 knee rating tools (46% general, 54% specific) across 29,742 patients.
Methodological quality varied, with patient selection and index test applicability scoring well, while construct validity showed higher quality.
Specific tools often target prosthesis, patellofemoral, or anterior cruciate ligament injuries, focusing on function, pain, and activity.

Conclusions:

A comprehensive compilation of validated knee assessment tools was created, despite limitations in validation quality.
The Copenhagen Knee Range of Motion Scale (osteoarthritis, arthroplasties), Knee Outcome Survey (general), and Lysholm Knee Score (general) demonstrated strong validation.
The Tegner Activity Score is recommended for anterior cruciate ligament injuries, with an operational document aiding tool selection for interventions.
