Background:

Shiitake mushrooms (Lentinula edodes) are a popular edible fungus.
Adverse skin reactions to shiitake mushrooms are rare but documented.
Flagellate dermatitis is a distinctive rash pattern characterized by linear or whip-like lesions.

Observation:

A previously healthy 37-year-old man developed a pruritic, erythematous, and edematous rash.
The rash appeared 48 hours after consuming raw shiitake mushrooms.
The lesions exhibited a characteristic flagellate (whip-like) morphology.

Findings:

The patient was diagnosed with flagellate dermatitis, commonly referred to as shiitake dermatitis.
Histopathological examination revealed a superficial perivascular dermatitis.
The condition is thought to be an immune response to lentinan, a polysaccharide in shiitake mushrooms.

Implications:

This case highlights the potential for shiitake mushrooms to induce flagellate dermatitis.
Prompt diagnosis and symptomatic treatment with antihistamines and topical corticosteroids are effective.
