Precision of Medication Therapy Problem Identification and Classification amongst Primary Care Clinic Pharmacists

Pharmacists showed moderate agreement in identifying medication therapy problems (MTPs) after implementing comprehensive medication management (CMM) services. Further training is recommended to enhance consistency in MTP identification and classification.
