Testosterone therapy (TTh) significantly improves erectile function in hypogonadal men. Benefits were most pronounced with testosterone gel, longer treatment durations, and in men without diabetes or metabolic syndrome.
Area of Science:
- Endocrinology
- Urology
- Men's Health
Background:
- Late-onset hypogonadism affects erectile function.
- Testosterone therapy (TTh) is a potential treatment.
- Evidence on TTh efficacy for erectile function requires systematic review.
Approach:
- Systematic review and meta-analysis of randomized controlled trials (RCTs) published since 2007.
- Included studies assessed erectile function using the International Index of Erectile Function-Erectile Function Domain (IIEF-EFD).
- Analyzed data from 18 RCTs (systematic review) and 6 RCTs (meta-analysis) involving 1,458 patients.
Key Points:
- TTh significantly improved IIEF-EFD scores compared to placebo (mean difference 1.86, p < 0.0001).
- Improvements were notable with testosterone gel, treatment exceeding 30 weeks, and in patients without diabetes mellitus or metabolic syndrome.
- Meta-analysis included 6 studies with 1,458 patients.
Conclusions:
- Testosterone therapy effectively enhances erectile function in hypogonadal men.
- Optimal outcomes observed with specific formulations (testosterone gel), treatment duration (>30 weeks), and in healthier patient subgroups.
- TTh offers a viable therapeutic option for improving sexual health in men with hypogonadism.
