    Summary

Persistent generalized lymphadenopathy (PGL) in intravenous drug abusers shows distinct abnormalities on lymphangiography and CT scans. These imaging findings are crucial for diagnosing lymphoma and monitoring disease progression.

    Area of Science:

    • Radiology
    • Oncology
    • Infectious Disease

    Background:

    • Intravenous drug abusers are at increased risk for persistent generalized lymphadenopathy (PGL).
    • PGL can be a precursor to malignant lymphoma, necessitating accurate diagnostic and staging methods.

    Purpose of the Study:

    • To evaluate the utility of lymphangiography (LAG) and abdominal computed tomography (CT) in characterizing PGL in intravenous drug abusers.
    • To assess the imaging findings associated with PGL and their potential to mimic or indicate lymphoma.

    Main Methods:

    • Retrospective review of 13 intravenous drug abusers with PGL.
    • Performance of lymphangiography (LAG) and abdominal computed tomography (CT) on all patients.
    • Analysis of imaging findings, including lymph node abnormalities, spleen size, and patterns suggestive of lymphoma.

    Related Experiment Videos

    Main Results:

    • All 12 LAG examinations revealed diffuse abnormalities with filling defects in pelvic and abdominal lymph nodes.
    • CT scans confirmed lymphadenopathy in 92% of patients, with retroperitoneal nodes consistently affected.
    • Splenomegaly was observed in 69% of patients.
    • LAG findings in two patients initially misdiagnosed with lymphoma were consistent with actual lymphoma involvement.

    Conclusions:

    • LAG and CT demonstrate characteristic patterns in PGL among intravenous drug abusers.
    • These imaging modalities are valuable for diagnosing lymphoma and staging patients with PGL, especially given the risk of malignant transformation.
