The cochlea is a coiled structure in the inner ear that contains hair cells—the sensory receptors of the auditory system. Sound waves are transmitted to the cochlea by small bones attached to the eardrum called the ossicles, which vibrate the oval window that leads to the inner ear. This causes fluid in the chambers of the cochlea to move, vibrating the basilar membrane.

Hair cells are the sensory receptors of the auditory system—they transduce mechanical sound waves into electrical energy that the nervous system can understand. Hair cells are located in the organ of Corti within the cochlea of the inner ear, between the basilar and tectorial membranes. The actual sensory receptors are called inner hair cells. The outer hair cells serve other functions, such as sound amplification in the cochlea, and are not discussed in detail here.

    Background:

    • The Electrical Cochlear Response (ECR) is an objective scalp potential for assessing cochlear implant (CI) function.
    • Objective adaptation of CI devices is crucial for individual patient needs.
    • Understanding ECR consistency across different CI designs is necessary.

    Purpose of the Study:

    • To investigate the consistency of Electrical Cochlear Response (ECR) wave shape morphology across different cochlear implant (CI) manufacturers.
    • To determine if CI design differences impact the objective ECR measure.
    • To validate ECR as a universal method for CI programming.

    Main Methods:

    • ECR was recorded from six contiguous intracochlear electrodes in twelve pediatric CI users, grouped by CI manufacturer.
    • Artifacts from CI stimulation were removed using Empirical Mode Decomposition.
    • ECR amplitude normalization and correlation analyses were performed to compare wave shape morphology.

    Main Results:

    • ECR wave shape morphology demonstrated consistency across different CI manufacturers.
    • Intra- and inter-group correlation coefficients for ECR ranged from 0.58 to 0.9 and 0.63 to 0.85, respectively.
    • Correlation decreased between apical and adjacent electrodes within the same patient and group.

    Conclusions:

    • ECR wave shape morphology is not significantly affected by differences in CI design or manufacturer.
    • ECR provides a reliable, objective, and manufacturer-independent methodology for CI programming and adjustment.
    • This objective measure is compatible with all current CIs without special hardware or software, enabling real-time assessment.
