Summary

Corticosteroids may reduce mortality in severe community-acquired pneumonia (CAP) patients with septic shock or high inflammation requiring mechanical ventilation. No survival benefit was seen in less severe CAP cases.

Purpose of the Study:

  • To investigate the efficacy of corticosteroids in improving clinical outcomes for severe CAP patients with heightened inflammatory responses.
  • To determine if corticosteroid use impacts mortality in severe CAP based on disease severity and inflammatory markers.

Main Methods:

  • Analysis of two prospective observational cohorts of severe CAP patients in Barcelona and Rome.
  • Propensity score (PS) matching to balance baseline characteristics between corticosteroid and non-corticosteroid groups.
  • Exclusion of patients with viral pneumonia or those treated with hydrocortisone.

  • Corticosteroid therapy was associated with lower 28-day mortality in severe CAP patients meeting major criteria (e.g., septic shock, mechanical ventilation).

  • No significant mortality benefit was observed in severe CAP patients not meeting major severity criteria.

Conclusions:

  • Corticosteroid treatment appears beneficial for severe CAP patients experiencing septic shock and/or requiring invasive mechanical ventilation.
  • The use of corticosteroids did not demonstrate a mortality benefit in severe CAP patients without these critical features.
