    Summary

    Patch testing revealed that DMDM hydantoin and Kathon CG are common causes of allergic reactions in suspected contact dermatitis patients. Dermatologists should consider adding Kathon CG to standard patch test series.

    Background:

    • Preservatives are widely used in consumer products.
    • Allergic contact dermatitis (ACD) is a significant health concern.
    • Routine patch testing aids in diagnosing ACD triggers.

    Purpose of the Study:

    • To evaluate the prevalence of allergic reactions to 14 common preservatives.
    • To determine if specific preservatives warrant inclusion in standard patch testing protocols.
    • To assess the diagnostic utility of certain preservatives for contact dermatitis.

    Main Methods:

    • A tray of 14 preservatives was applied to 501 consecutive patients with suspected contact dermatitis.
    • Patch test results were analyzed to identify preservatives eliciting >1% positive reactions.
    • Reactions to specific allergens like formaldehyde and formaldehyde-releasers were investigated.

    Main Results:

    • DMDM hydantoin, Kathon CG, and alkyl trimethyl ammonium chloride showed >1% positive reactions.
    • Four out of six patients reacting to DMDM hydantoin also reacted to formaldehyde.
    • Alkyl trimethyl ammonium chloride at 0.1% in aqua was deemed too concentrated.

    Conclusions:

    • Kathon CG is identified as a potentially significant allergen in the Netherlands.
    • Dermatologists in the Netherlands may benefit from adding Kathon CG to their standard patch test series.
    • Quaternium-15 inclusion in the standard series showed limited value for this patient group.
