Summary

This meta-analysis found no significant difference in major adverse cardiac events (MACE) between one-stent and two-stent approaches for percutaneous coronary intervention (PCI) bifurcation lesions. However, the double kissing crush technique showed promising results for reducing MACE and other adverse events.

Background:

  • Percutaneous coronary intervention (PCI) bifurcation techniques have shown varied efficacy and safety outcomes.
  • Optimizing PCI for bifurcation lesions remains a challenge in interventional cardiology.

Purpose of the Study:

  • To compare the efficacy and safety of one-stent versus two-stent PCI bifurcation approaches.
  • To evaluate different bifurcation techniques using network meta-analysis.

Main Methods:

  • Systematic electronic search of Medline, Scopus, and Cochrane databases for randomized controlled trials.
  • Pairwise meta-analysis comparing one-stent vs. two-stent approaches.
  • Network meta-analysis comparing various bifurcation techniques.

Main Results:

Conclusions:

  • One-stent and two-stent PCI bifurcation strategies show comparable long-term MACE risk.
  • The double kissing crush technique appears superior among evaluated bifurcation techniques, associated with reduced MACE and target lesion revascularization.
