    Summary

    Diagnosing endogenous fungal eye infections is challenging. Early diagnosis via vitreous biopsy and combined surgical-antifungal treatment are crucial for cryptococcal endophthalmitis.

    Background:

    • Endogenous fungal endophthalmitis poses diagnostic challenges.
    • Intraocular inflammation unresponsive to standard therapies requires further investigation.

    Observation:

    • A patient presented with hydrocephalus, progressing to bilateral intraocular inflammation.
    • Diagnosis was confirmed as cryptococcal chorioretinitis, endophthalmitis, and meningitis via vitreous biopsy and lumbar puncture.

    Findings:

    • Systemic amphotericin monotherapy failed to resolve intraocular fungal infection.
    • Vitrectomy combined with intravitreal and systemic amphotericin successfully halted disease progression.

    Implications:

    • Vitreous biopsy is valuable for diagnosing refractory intraocular inflammation.
    • Combined surgical and intravitreal antifungal therapy is effective for severe fungal endophthalmitis.
