Background:

Purpose of the Study:

  • To investigate the incidence and clinical characteristics of chemotherapy-induced lung injury.
  • To identify common offending agents and evaluate treatment outcomes.

Main Methods:

  • A 3-year prospective observational study of 624 chemotherapy patients.
  • Evaluation of 18 patients with nonresolving respiratory symptoms using CT scans, DLCO, bronchoscopy, and biopsy.
  • Exclusion of pulmonary infections prior to diagnostic workup.

Main Results:

  • An incidence of 2.8% for chemotherapy-induced lung injury was observed.
  • Ground-glass opacities were the most common CT finding; organizing pneumonia was the most frequent biopsy result.
  • Drug discontinuation was effective in 15 patients; 3 required oral corticosteroids.

Conclusions:

  • Chemotherapy-induced lung injury presents with diverse pulmonary involvement.
  • Prompt recognition and management, including drug withdrawal and possibly corticosteroids, are key to favorable outcomes.
