  • Initial laboratory results showed significantly prolonged prothrombin time (PT), international normalized ratio (INR), and partial thromboplastin time (PTT).
  • Coagulation assays confirmed VKD, with low Factor VII and Factor IX levels, despite adequate caloric intake from a home blenderized diet.

Findings:

  • The patient's blenderized tube feeding regimen failed to meet the adequate intake (AI) for vitamin K.
  • Intravenous vitamin K (phytonadione) administration for five days successfully corrected the coagulopathy, normalizing PT, PTT, and INR.
  • Dietary review revealed insufficient vitamin K and other minerals in the blenderized diet.

Implications:

  • Clinicians must consider VKD in patients on blenderized tube feeds, as these diets may not meet vitamin K requirements.
  • Monitoring of macro and micronutrients is crucial for patients on non-standard or inadequate enteral nutrition.
  • Prompt diagnosis and supplementation of vitamin K are vital for managing VKD and preventing bleeding complications in at-risk populations.
