  • Alpha-1 antitrypsin deficiency is a genetic condition that can lead to bullous emphysema, a severe lung disease.

Observation:

  • A 29-year-old pregnant patient with alpha-1 antitrypsin deficiency and bullous emphysema experienced a mid-trimester pneumothorax requiring chest tube intervention.
  • The patient successfully delivered a healthy infant via vaginal delivery under epidural anesthesia with midforceps assistance.

Findings:

  • Despite a serious pulmonary complication during pregnancy, a viable delivery was achieved.
  • This case suggests that vaginal delivery might be feasible in select multiparous patients with bullous emphysema.

Implications:

  • Physicians should consider individualized risk-benefit assessments for pregnant patients with severe lung disease.
  • Vaginal delivery should not be automatically contraindicated in multiparous women with bullous emphysema, even with a history of pneumothorax.
