Background:

  • Motor vehicle collisions (MVCs) are a leading cause of unintentional death in children and adolescents.
  • Public awareness and adherence to child restraint recommendations are insufficient.
  • Child safety in vehicles remains a critical public health concern.

Purpose of the Study:

  • To investigate the utilization of child safety restraints in pediatric MVCs.
  • To examine the association between restraint use and patient outcomes.
  • To identify disparities in restraint use among different demographic groups.

Main Methods:

  • Retrospective analysis of a level 1 trauma registry for pediatric MVC patients (0-18 years) from October 2013 to December 2018.
  • Collection and examination of demographic and clinical variables, including restraint use by age group.
  • Statistical analysis to compare outcomes based on restraint status and demographics.

Main Results:

  • Over half (53%) of pediatric MVC patients were improperly restrained or unrestrained.
  • High rates of improper restraint were observed across all age groups, including car seat (62%) and booster seat (51%) users.
  • Improperly restrained children experienced higher injury severity, increased need for operative intervention, and higher rates of discharge to rehabilitation facilities. Adolescent mortality was higher among unrestrained individuals.

  • Despite increased efforts, a significant proportion of children remain improperly restrained during MVCs.
  • Targeted injury prevention services and community outreach are essential to educate vulnerable populations, particularly those with infants and toddlers, on proper vehicle safety.
  • Addressing disparities in restraint use among different racial and ethnic groups is critical for reducing preventable injuries and fatalities.
