Background:

Primary Breast Sarcomas (PBS) are rare, accounting for less than 1% of all breast malignancies.
These non-epithelial tumors arise from mesenchymal mammary tissue and can be challenging to distinguish from other breast sarcomas.

Observation:

A retrospective study reviewed 1570 breast malignancy cases over five years, identifying 5 instances of PBS (0.32%).
The study observed a higher incidence in males (60%) compared to females (40%), with ages ranging from 32 to 65 years (mean 48.5).
Diagnosed subtypes included Malignant Peripheral Nerve Sheath Tumors (MPNST), Dermatofibrosarcoma Protuberans (DFSP) with fibrosarcoma, Leiomyosarcoma, and Fibrosarcoma.

Findings:

Primary Breast Sarcomas are exceptionally rare and demonstrate local aggressiveness.
Accurate histopathological and immunohistochemical (IHC) diagnosis is essential for appropriate management.

Implications:

Recognizing PBS is critical due to distinct treatment protocols compared to epithelial breast cancers.
This highlights the need for specialized diagnostic approaches for rare breast malignancies.
