Primary adrenal diffuse large B-cell lymphoma is rare. While R-CHOP is the standard treatment, surgery aids diagnosis, but optimal treatment remains uncertain.

- The patient underwent left adrenal gland resection followed by R-CHOP immunochemotherapy.
- Despite treatment, the patient's condition worsened, leading to death from respiratory failure.
- R-CHOP is the current standard immunochemotherapy for primary adrenal DLBCL.
