Summary
Intentional replantation (IR) is a viable endodontic treatment for root canal failure when conventional retreatment or surgery is not feasible. This case study shows successful healing and bone regeneration after IR of a mandibular premolar.
Area of Science:
- Endodontics
- Oral Surgery
Background:
- Root canal treatment failure necessitates retreatment or alternative procedures.
- Removal of post and core can be challenging, limiting access for conventional retreatment.
- Intentional replantation (IR) offers an alternative when standard endodontic retreatment or apical surgery is contraindicated.
Observation:
- A case of intentional replantation (IR) was performed on a right mandibular second premolar (#45) due to inability to remove a post and core.
- Micro-apical surgery was considered but IR was chosen as the alternative procedure.
- Postoperative evaluation at six months showed no symptoms, uneventful healing, and significant bone regeneration.
Findings:
- Intentional replantation demonstrated short-term clinical efficacy in this case.
- Successful bone regeneration was observed around the replanted tooth.
- The procedure was well-tolerated with no significant discomfort.
Implications:
- IR can be an accepted endodontic treatment option for root canal treatment failure.
- It is particularly useful when anatomical limitations preclude apical surgical access.
- This case supports IR as a conservative alternative to extraction in complex endodontic situations.
