    Background:

    • Congenital hydronephrosis, or pyeloureteral junction obstruction (PUJO), is often treated with pyeloplasty.
    • Surgical options include open, laparoscopic, and robotic approaches in pediatric patients.

    Purpose of the Study:

    • To analyze a pilot cohort of pediatric patients undergoing primary robotic pyeloplasty.
    • To evaluate the safety and efficacy of robotic pyeloplasty in treating PUJO.

    Main Methods:

    • Retrospective analysis of 26 pediatric patients (ages 3-17) who underwent robotic pyeloplasty between December 2018 and June 2021.
    • Exclusion of patients with prior PUJO surgery.
    • Evaluation of indication criteria, symptoms, operative time, hospitalization, and complications.

    Main Results:

    • The most common indication was an aberrant vessel (85%).
    • Postoperative ultrasound showed regression of pelvic dilatation in 92% of patients.
    • A 27% complication rate (Clavien-Dindo 3b) was observed, with 2 patients requiring reoperation.

    Conclusions:

    • Robotic pyeloplasty is a viable alternative to traditional surgical methods for pediatric PUJO.
    • The high complication rate in this early cohort may be attributed to the novelty of the technique and the need for further experience.
