Primary Diffuse Large B Cell Lymphoma of Bone: A Single-Center Experience.

Background:

• Primary diffuse large B-cell lymphoma of the bone (PB-DLBCL) is an uncommon extranodal lymphoma.
• This study focuses on adult Jordanian patients diagnosed with PB-DLBCL.

Purpose of the Study:

• To investigate the clinical characteristics of PB-DLBCL in Jordanian adults.
• To analyze treatment modalities, patient outcomes, and the risk of central nervous system relapse (CNSR).

Main Methods:

• Retrospective analysis of PB-DLBCL patients (age >16) treated between 2002 and 2021.
• Patients were classified as unifocal (UF) or multifocal (MF) based on the number of bone sites involved.
• Data on clinical features, treatments, outcomes, and CNSR were extracted and analyzed.

• Twelve adult patients were identified with PB-DLBCL; the vertebral column was the most common site.
• Unifocal PB-DLBCL (n=8) showed a good prognosis with complete remission (CR) in most cases.
• Multifocal PB-DLBCL (n=4) was associated with a high International Prognostic Index score, increased CNSR risk, and short survival, with no CR achieved.

Conclusions:

• PB-DLBCL is a rare entity in adult Jordanian patients, with unifocal presentation being more frequent.
• Unifocal PB-DLBCL generally carries a favorable prognosis.
• Multifocal PB-DLBCL presents a significant challenge due to poor prognosis, high CNSR risk, and limited survival.
