Summary

Pentosan polysulfate sodium (PPS) maculopathy can progress rapidly over two years, even without initial signs. Early detection and stopping PPS therapy are crucial to prevent vision loss from this macular disease.

Background:

Pentosan polysulfate sodium (PPS) is used for various conditions.
Maculopathy is a potential adverse effect of long-term PPS exposure.
Early detection is key to managing PPS toxicity.

Observation:

An elderly patient on long-term PPS therapy (1,205 g cumulative dose) was evaluated.
Multimodal retinal imaging was performed at baseline and after 2 years.
Initial imaging showed no significant signs of PPS maculopathy.

Findings:

After 2 years, advanced PPS maculopathy was detected in both eyes.
Rapid progression of macular changes was evident on multimodal imaging.
Spectral domain optical coherence tomography revealed significant toxicity.

Implications:

This case highlights the potential for rapid progression of PPS maculopathy.
Early and regular screening for PPS toxicity is essential for patients on long-term therapy.
Prompt discontinuation of PPS may help slow or halt disease progression.
