Background:

Recreational SCUBA diving is a popular activity with millions of certified divers in the U.S.
Decompression sickness (DCS), or
the bends,
is a common diving-related injury, typically causing joint pain within 24 hours of surfacing.

Observation:

A 35-year-old male diver experienced initial resolution of joint pain following type 1 DCS.
He subsequently developed severe right knee pain and swelling after a strenuous lower body workout.
Standard imaging was unremarkable, and conservative treatments failed.

Findings:

Ultrasound-guided aspiration of the right knee joint was performed.
This procedure led to complete resolution of the patient's symptoms.
This suggests a potential recurrence or atypical presentation of DCS.

Implications:

Clinicians should consider delayed or recurrent DCS manifestations in divers presenting with joint pain after initial recovery.
Ultrasound-guided aspiration may be a beneficial treatment for suspected recurrent DCS joint pain.
