Home-based care models are diverse, necessitating a clear definition of professional nursing practice.
Existing frameworks may not fully capture the unique aspects of nursing within the patient's home environment.

Purpose of the Study:

To introduce a descriptive, middle-range theoretical framework specifically for professional nursing practice in the home.
To define key concepts and relationships relevant to home-based nursing care.

Main Methods:

Development of a theoretical framework based on descriptive analysis.
Identification of six core concepts: primacy of home, patient authority, patient self-management, caregivers as collaborators, interprofessional clinical team collaboration, and nurse autonomy.
Formulation of four relational statements describing patient and provider experiences in the home.

Main Results:

The framework integrates six concepts and four relational statements to describe home-based nursing practice.
Key theoretical statements highlight the influence of home environment, patient decision-making, interprofessional collaboration, and nurse autonomy.

Conclusions:

This theoretical framework can guide nursing education, organizational structures, and practice in home-based care.
It aims to maintain the integrity and purpose of the professional nurse's role in home settings.
