    Summary

    Reconstructing retaining ligaments during SMAS facelifts significantly reduces early signs of facial aging relapse. While this technique improves facelift fixation, it carries a slightly higher risk of temporary nerve paralysis.

    Background:

    • Effective sub-superficial musculoaponeurotic system (SMAS) facelifts require full release of retaining ligaments.
    • Standard fixation methods may lead to early relapse of facial aging signs due to premature failure.

    Purpose of the Study:

    • To evaluate the efficacy of a novel ligament reconstruction technique in reducing early relapse rates after facelift surgery.
    • To assess the impact of this technique on complication rates, specifically temporary nerve paralysis.

    Main Methods:

    • A comparative analysis of 188 consecutive facelift cases.
    • 104 patients underwent standard fixation (control group), while the remaining had ligament reconstruction.
    • Early relapse was diagnosed using a novel set of criteria based on the Merz scale.

    Main Results:

    • The ligament reconstruction group showed a significantly lower early relapse rate (0.53%) compared to the control group (5.76%).
    • An increased incidence of temporary nerve paralysis (8.5%) was observed in the ligament reconstruction group.

    Conclusions:

    • Sub-SMAS reconstruction of retaining ligaments effectively enhances facelift fixation and decreases early relapse.
    • The trade-off for reduced relapse is an acceptable increase in the risk of temporary neuropraxia.
