Background:

  • Wilms' tumour, a rare pediatric kidney cancer, can exceptionally involve the spinal cord.
  • Malignant spinal cord compression presents a critical oncologic emergency.

Observation:

  • A 14-year-old female presented with an abdominal mass, paraplegia, and loss of bladder/bowel control.
  • Radiology revealed a large intraabdominal mass infiltrating the spinal canal, compressing the spinal cord and nerve roots.

Findings:

  • Histopathology confirmed the diagnosis as nephroblastoma (Wilms' tumour).
  • The patient demised before neoadjuvant chemotherapy could be initiated.

Implications:

  • This case highlights the critical need for early diagnosis of spinal cord compression in pediatric oncology.
