Summary

Boerhaave

Background:

  • Boerhaave's syndrome, a rare esophageal perforation, presents diagnostic challenges due to non-specific symptoms.
  • High mortality necessitates prompt diagnosis and intervention.

Observation:

  • A 63-year-old male presented with respiratory distress, bilateral tension hydropneumothorax, gastric distention, and aerobilia on CT.
  • Subsequent CT confirmed esophageal perforation with pleural contrast extravasation.

Findings:

  • Immediate thoracic drainage and urgent surgical repair were performed.
  • The patient developed septic shock, requiring intensive care unit (ICU) admission.

Implications:

  • Early diagnosis and a damage control strategy, including thoracic drainage and surgery, are crucial.
