Differentiating Between Acute Decompensated Aortic Stenosis and Myocardial Infarction.

In patients with severe aortic stenosis (AS) presenting acutely, elevated Troponin T (TnT), ischemic ECG, and angina are common but do not reliably diagnose type 1 myocardial infarction (NSTEMI). Definitive diagnosis requires coronary imaging.

Background:

- Differentiating these conditions is critical for appropriate management and patient outcomes.
Purpose of the Study:
- To assess the diagnostic accuracy of Troponin T (TnT), ischemic ECG, and angina in predicting type 1 NSTEMI and obstructive coronary artery disease (CAD) in severe AS patients with acute presentations.
- To evaluate the utility of these markers in distinguishing type 1 NSTEMI from acute decompensated AS.

- Inclusion of patients with severe AS and acute symptoms (angina, dyspnea, syncope).
- Endpoints defined as type 1 NSTEMI (coronary thrombus or >90% stenosis) and obstructive CAD (>70% stenosis).

- These markers showed low sensitivity (18-41%) but high specificity (57-77%) for type 1 NSTEMI and obstructive CAD.
Conclusions:
- Angina, ischemic ECG, and elevated TnT are frequent in acute AS presentations but are not specific for type 1 NSTEMI.
- These clinical and biomarker findings alone cannot reliably differentiate type 1 NSTEMI from acute decompensated AS.
- Coronary imaging (CT or ICA) is essential for definitive type 1 NSTEMI diagnosis in severe AS patients.
