Triggered rhythms in atrial muscle.

S L Lipsius

    Journal of Electrocardiology
    |January 1, 1987

    Summary

Norepinephrine triggers arrhythmias in dog heart tissue through delayed after-depolarizations. These triggered rhythms can exhibit abrupt changes in cycle length and configuration, suggesting multiple pacemaker sites.

    Area of Science:

    • Cardiology
    • Electrophysiology
    • Cardiac Arrhythmias

    Background:

    • Triggered activity is a key mechanism in cardiac arrhythmias.
    • Delayed after-depolarizations (DADs) can initiate triggered beats.
    • Norepinephrine is known to influence cardiac electrophysiology.

    Purpose of the Study:

    • To investigate triggered activity in canine right atrial tissue.
    • To characterize the patterns and mechanisms of triggered arrhythmias induced by norepinephrine.
    • To explore the role of abrupt cycle length changes in triggered rhythms.

    Main Methods:

    • Isolated canine inferior right atrial tissues were used.
    • Intracellular and extracellular recordings were employed.
    • Norepinephrine (NE) was administered at concentrations > 10(-7) M.
    • Stimuli and rapid pacing protocols were applied.

    Main Results:

    • Norepinephrine induced action potentials with DADs, leading to non-driven beats.
    • Sustained triggered activity often showed stable configurations but gradual cycle length changes.
    • Some preparations exhibited abrupt shifts in configuration and cycle length during triggered activity.
    • Pacing during triggered activity could induce abrupt cycle length decreases and configuration changes.

    Conclusions:

    • Abrupt changes in cycle length and configuration can occur during triggered activity.
    • These dynamic changes suggest interactions between multiple triggered pacemaker sites.
    • This phenomenon may underlie unique patterns of cardiac arrhythmias.
