Venous thoracic outlet syndrome: When exercising may be discouraged

Thoracic outlet syndrome (TOS) can cause deep vein thrombosis (DVT) in young adults. Early diagnosis of venous TOS is crucial to prevent serious complications.

- Thoracic outlet syndrome (TOS) is caused by neurovascular compression.
- TOS presents with subtypes: neurogenic, venous, or arterial, based on the compressed structure.
- Neurogenic TOS is most common (90%), while vascular TOS is rare.

- Two case reports of young patients with upper extremity deep vein thrombosis (DVT) are presented.
- Both cases involved venous thoracic outlet syndrome (VTOS).
- One VTOS case stemmed from an anatomical variant; the other, an effort thrombosis from exercise.
Findings:
- Venous thoracic outlet syndrome can manifest as upper extremity DVT in young individuals.
- Anatomical variations and repetitive arm movements are potential causes of VTOS-induced DVT.
- These cases highlight uncommon presentations of a rare condition.
Implications:
- Timely identification and treatment of venous TOS are essential to avoid significant patient morbidity.
- Increased awareness of VTOS is needed, particularly in young patients presenting with unexplained DVT.
- Diagnostic protocols may need refinement to include TOS evaluation for relevant DVT cases.
