    Summary

Textured breast implants are safe and effective, showing no significant difference in complications or scar malposition compared to smooth implants. Micro-textured implants may offer a trend toward reduced scar malposition, even in high-risk patients.

    Area of Science:

    • Plastic Surgery
    • Biomaterials Science
    • Oncology

    Background:

    • Textured breast implants historically reduce malposition and capsular contracture.
    • Recent concerns link textured devices to breast implant-associated anaplastic large cell lymphoma (BIA-ALCL), prompting reevaluation.
    • This study addresses the safety and efficacy of textured implants amidst these concerns.

    Purpose of the Study:

    • To develop an algorithm for selecting between smooth and micro-textured breast implants.
    • To provide clinical rationale for the beneficial use of micro-textured implants.
    • To compare complication and malposition rates between smooth and micro-textured implants.

    Main Methods:

    • A retrospective review of 133 primary augmentation patients (Jan 2018-Dec 2020).

  • Comparison of 84 patients with smooth implants versus 49 patients with micro-textured implants.
  • All surgeries performed in dual plane via inframammary incision; complications and scar malposition were recorded.
  • Main Results:

    • No significant difference in implant-related complications (3.57% smooth vs. 2.04% micro-textured).
    • No cases of BIA-ALCL were reported in either group.
    • Scar malposition rates were not significantly different (15.4% smooth vs. 8.16% micro-textured), despite higher risk factors in the micro-textured group.

    Conclusions:

    • Micro-textured breast implants remain a safe and effective option.
    • A trend towards decreased scar malposition with micro-textured implants was observed, though not statistically significant.
    • High-risk patients for malposition using micro-textured implants achieved results comparable to average-risk patients using smooth implants.
