Summary

Daily hypertonic saline nasal irrigation is effective for pediatric chronic rhinosinusitis (CRS). Xylitol irrigation showed poor tolerance and is not recommended for children with CRS.

Background:

Purpose of the Study:

  • To compare the efficacy and outcomes of daily hypertonic saline irrigation versus saline/xylitol irrigation in treating pediatric CRS.
  • To assess the impact of these treatments on patients' quality of life.

Main Methods:

  • A prospective, randomized, single-blinded study involving 125 children diagnosed with CRS.
  • Participants received twice-daily intranasal irrigations with either hypertonic saline or saline/xylitol for six weeks.
  • Treatment outcomes were measured using the Sinonasal Quality of Life Survey (SN-5) at baseline and after six weeks.

Main Results:

  • Hypertonic saline irrigation led to statistically significant improvements in four domains of the SN-5 score and overall quality of life compared to baseline.

Conclusions:

  • Xylitol irrigation is not recommended as a first-line treatment for pediatric CRS due to low tolerance, compliance, and side effects.
  • Twice-daily, large-volume, low-pressure hypertonic saline nasal irrigation for six weeks is a safe and effective baseline treatment for pediatric CRS.
  • Hypertonic saline irrigation can be considered a foundational therapy before surgical interventions for pediatric CRS.
