Summary

Ertapenem prophylaxis may be effective for obese patients undergoing shorter surgeries. A single 1g dose demonstrated adequate drug concentrations for procedures up to four hours, particularly for susceptible bacteria.

Background:

  • Surgical site infections (SSIs) are a significant concern, especially in obese patients.
  • Ertapenem is a common antibiotic for surgical prophylaxis in gastrointestinal surgeries.

Purpose of the Study:

  • To characterize the population pharmacokinetics of ertapenem for surgical prophylaxis.
  • To evaluate the probability of target attainment (PTA) for ertapenem in obese surgical patients.

Main Methods:

  • A population pharmacokinetic model was developed from plasma samples of 10 patients receiving a single 1g intravenous dose of ertapenem.
  • Monte Carlo simulations were used to predict PTA for various minimum inhibitory concentrations (MICs).

Main Results:

  • The pharmacokinetic parameters included a half-life of 4.3 hours.
  • PTA exceeded 90% for MICs ≤0.125 mcg/mL for surgeries up to 4 hours.
  • Adequate PTA was not achieved for MICs ≥0.5 mcg/mL.
