This case report highlights the successful use of shockwave intravascular lithotripsy (IVL) for managing complex coronary artery calcification in India. IVL-assisted percutaneous coronary intervention (PCI) effectively treated severe right coronary artery stenosis.

Area of Science:

  • Cardiology
  • Interventional Cardiology

Background:

  • Coronary artery calcification poses significant challenges during percutaneous coronary intervention (PCI).
  • Shockwave intravascular lithotripsy (IVL) has emerged as a valuable tool for treating calcified coronary lesions.
  • There is limited published data on IVL use in the Indian population.

Observation:

  • A 74-year-old male presented with ST-segment elevation myocardial infarction (STEMI).
  • Coronary angiography revealed thrombotic occlusion in the left anterior descending artery (LAD) and calcific stenosis in the right coronary artery (RCA).
  • The patient underwent immediate PCI for LAD and a staged PCI for RCA due to unstable angina.

Findings:

  • The staged PCI of the RCA was successfully performed with shockwave IVL assistance.
  • The procedure effectively managed the severe calcific stenosis in the RCA.
  • Post-intervention, the patient showed improved thrombolysis in myocardial infarction (TIMI) flow.

Implications:

  • This case demonstrates the feasibility and efficacy of shockwave IVL in managing complex coronary artery calcification in an Indian patient.
  • It supports the use of IVL as a treatment option for challenging calcified lesions in this demographic.
  • Further studies are warranted to establish IVL's role in the Indian interventional cardiology landscape.

Coronary Artery Disease V: Interprofessional Care

Interprofessional care for coronary artery disease includes pharmacological therapy and revascularization procedures.Pharmacological therapy for Coronary Artery Disease (CAD) aims to manage symptoms, prevent complications, and improve patient outcomes through various classes of medications:Antiplatelet Agents:Aspirin and Clopidogrel: These medications inhibit platelet aggregation, preventing blood clots, which is crucial for avoiding heart attacks and strokes. Doctors often prescribe these...

Acute Coronary Syndrome IV: Interprofessional Care

IntroductionThe management of Acute Coronary Syndrome (ACS) aims to minimize myocardial damage, preserve myocardial function, and prevent complications.Initial ManagementInpatient management involves continuous cardiac monitoring, preferably in an ICU, focusing on blood pressure, serum sodium, potassium, and creatinine levels, and urine output. Ongoing pharmacologic management is crucial for stabilizing the patient.Supplemental Oxygen: Administer supplemental oxygen if oxygen saturation is...

Acute Coronary Syndrome I: Introduction

Acute Coronary Syndrome (ACS) encompasses a spectrum of heart conditions caused by sudden obstruction of coronary arteries, typically resulting from the rupture of an atherosclerotic plaque and subsequent thrombus (blood clot) formation. This obstruction can lead to partial or complete blockage of blood flow, causing varying degrees of myocardial ischemia or infarction.ACS includes the following clinical entities:Unstable Angina (UA)Non-ST-Elevation Myocardial Infarction (NSTEMI)ST-Elevation...

Procedures for Kidney StonesMedical intervention is necessary when kidney stones or renal calculi are too large to pass spontaneously (typically greater than 5 millimeters) when stones are accompanied by symptomatic infection (such as fever or pyelonephritis), when they impair kidney function, or when they cause persistent symptoms like severe pain, nausea, or urinary retention. Additionally, patients with only one kidney or those who cannot be treated with medical management also require...

Peripheral Artery Disease III: Interprofessional Care

Peripheral Artery Disease (PAD) is characterized by narrowed arteries that diminish blood flow to the extremities. Effective management of PAD requires an interprofessional approach involving various healthcare professionals. The critical aspects of interprofessional care for PAD patients focus on risk factor modification, drug therapy, exercise therapy, nutrition therapy, critical limb ischemia care, and interventional radiology and surgical procedures. The primary treatment goal for PAD...

Cardiac Catheterization I: Pre-Procedure Overview

Cardiac catheterization is an invasive diagnostic technique used to identify and evaluate structural and functional diseases of the heart and major blood vessels. This technique diagnoses congenital heart disease, coronary artery disease, valvular heart disease, and coronary spasms and assesses ventricular function. It helps guide treatment decisions, including the need for revascularization procedures like percutaneous coronary intervention (PCI) or coronary artery bypass grafting (CABG) and...
