    Background:

    • Emphysematous bullae are common in chronic obstructive pulmonary disease (COPD).
    • Development of air-fluid levels within bullae can indicate complications.

    Purpose of the Study:

    • To investigate the clinical presentation, diagnosis, and treatment of air-fluid levels in emphysematous bullae.

    Main Methods:

    • Prospective identification and follow-up of 14 patients with air-fluid levels in emphysematous bullae.
    • Clinical evaluation, laboratory tests, fluid aspiration, and bronchoscopy were performed.
    • Treatment with oral penicillin or tetracycline was administered.

    Main Results:

    • Twelve of 14 patients presented with symptoms including fever, dyspnea, and cough.
    • Bacteroides melaninogenicus was identified in one patient's bullae aspirate.
    • All 13 treated patients achieved complete resolution of symptoms and radiographic findings within 2-32 weeks.

    Conclusions:

    • Air-fluid levels in emphysematous bullae often present with significant symptoms.
    • Antibiotic therapy is effective in resolving these complications.
