Background:

Danon syndrome is a rare genetic disorder affecting the heart.
Heart transplantation is a life-saving intervention for end-stage heart failure.
Pregnancy after heart transplantation presents unique challenges for maternal and fetal well-being.

Observation:

A 29-year-old woman with a history of heart transplant for Danon syndrome achieved a spontaneous pregnancy.
The fetus was confirmed not to have Danon syndrome.
Maternal echocardiography and fetal ultrasound monitoring throughout the pregnancy were reassuring.

Findings:

The patient developed severe pre-eclampsia at 36 weeks and 6 days gestation.
An emergency Cesarean section resulted in the delivery of a healthy male infant weighing 3,070g.
Postpartum, the mother experienced rapid normalization of blood pressure and was discharged on day four.

Implications:

This case highlights the feasibility of successful pregnancy in women with Danon syndrome post-heart transplant.
Careful multidisciplinary management is crucial for optimizing outcomes in pregnant heart transplant recipients.
Further research is warranted to understand the long-term implications for both mother and child.
