Pseudohypoparathyroidism (PHP) is a rare disorder causing low calcium and high phosphate due to parathyroid hormone resistance. This case highlights diagnostic challenges in a boy with PHP1B presenting with seizures and long QT syndrome.
Area of Science:
- Endocrinology
- Genetics
- Pediatrics
Background:
- Pseudohypoparathyroidism (PHP) is a rare genetic disorder characterized by resistance to parathyroid hormone (PTH), leading to hypocalcemia and hyperphosphatemia.
- PHP presents with diverse clinical, biochemical, and genetic features, often requiring complex diagnostic approaches.
Purpose of the Study:
- To report the clinical case of a boy with Pseudohypoparathyroidism type 1B (PHP1B).
- To highlight the diagnostic complexities in patients with elevated PTH, particularly when co-occurring with other conditions like long QT syndrome.
Main Methods:
- Detailed clinical history of a pediatric patient with PHP1B.
- Review of diagnostic workup including laboratory tests and molecular analyses.
- Management of acute symptomatic hypocalcemia and co-existing congenital long QT syndrome.
Main Results:
- The patient presented with a severe epileptic attack attributed to symptomatic acute hypocalcemia.
- The patient had a history of congenital long QT syndrome (LQTS) with a KCNQ1 gene mutation, treated with beta-blockers.
- Laboratory tests and molecular analyses were critical in diagnosing and managing the complex presentation.
Conclusions:
- This case underscores the diagnostic challenges in patients with elevated PTH and hypocalcemia.
- The importance of considering differential diagnoses and comprehensive evaluation in managing PHP is emphasized.
- Timely and accurate diagnosis is crucial for effective treatment of PHP and associated conditions.
